Depression and anxiety in people with long-term physical conditions

Taking the right course of action early is vital when a patient is suffering with mental health problems.

This is a decision support tool developed by the research team at the Faculty of Health and Life Sciences at Coventry University. It aims to support various clinicians to assess and refer onward any patient with a long term condition that they suspect of having anxiety or depression.

This tool guides the clinician through a questionnaire assessment to ascertain whether the patient is likely to have depression or anxiety, and provides guidance as to appropriate subsequent referral.  The tool also features further information on useful resources.

The tool is aimed at any clinician that is working closely and regularly with a patient that has a long term condition. It may be particularly useful for community matrons and specialist nurses, such as specialist cardiac nurses, that work closely with patients that have a long term condition such as heart failure. The tool is not suitable for use by patients.
