This 'step by step guide' has been created to help teams to develop local integrated care.  It starts with some key learning points from the experience of Vanguards and Pioneers, and suggests a simple project plan.

Experience from the Vanguards and Pioneers has shown that a good local plan, with strong local system leadership and determined engagement with colleagues and the local community, will deliver the most effective integrated care for local people.

National guidance in the Next Steps on the Five Year Forward View confirms that there is no ‘one size fits all’ way to do this.  Hence the guide does not aim to provide fixed answers,but asks questions to prompt local discussions.
