Self-Care Resources to Support Person Centred Care

It is widely recognised that those individuals who have access to appropriate information and understand the range of options available to them, make better informed decisions regarding self-care. Effective communication enables individuals to identify their strength, assess their needs, develop and gain the confidence to self-care. Empowering people to take responsibility for their own health and wellbeing can help them remain independent for longer.

Following a thorough review, a set of valuable tools and resources were identified by the Integrated Care Transformation Theme Programme of West Midlands to encourage self-care.

Dementia Road Map

A web based platform hosted by Dementia Pathfinders and developed in partnership with the Royal College of General Practitioners and the Department of Health. It provides high quality information about the dementia journey alongside other support groups and local services. It also supports the Dementia framework developed by Health Education England.

 

Common Core Principles to Support Self-care

This manual from Skills for Care and Skills for Health includes a series of 10 modules to support the delivery of learning interventions. 

 

Self-Care Champions

A self-care campaign from the Self Care Forum launched in 2010 and is made up pf professionals from key organisations across primary care including the NHS Alliance, Royal College of Nursing, National Association of Primary Care, National Association of Patient Participation and the Proprietary Association of Great Britain.

 

This Health Education England work programme focused on education and training to workforce in care homes, both residential and nursing, with the support of patient quality teams within seven clinical commissioning groups (CCGs) across Arden, Herefordshire and Worcestershire.

 

Carers are usually unpaid and include friends, family and volunteers.

 

FallsCheck App

A web-based app from Coventry University that helps prevents falls in the home.

 

Living Made Easy

Developed by the Disabled Living Foundation, it provides information and impartial advice about equipment for daily living. The product information is supported by expert advice created by a team of occupational therapists.

 

Altogether Better

An evidence based community health champion approach for working with citizens and services to improve health.

 

Social Media Surgeries

This is an informal gathering of people who want to learn how to use the web to communicate campaign or collaborate. No presentations, no jargon and no one telling people what they think they should know.

 

Carers Toolkit

An integrated approach to identifying and assessing carer health and wellbeing from NHS England and Partners, covering the new duties on NHS organisation as part of the Care Act 2014 and the Children and Families Act 2014. It gives examples of positive practice making a difference to carers and their families.

 

Patients and citizens include all the people who use health and social care services.

Award winning guided advice tool developed by the Disability Living Foundation charity, to help people to find solutions for independent living.

NHS Choices has interactive tools, apps and podcasts to help manage health and care.

 

AT Home

The AT Home campaign is a collaboration amongst the 14 Local Authorities covering the West Midlands, supported by companies who operate in this market. A campaign designed to inform people about the simple aids and equipment available to support self-care and self-management.

An awareness tool that showcases real life examples of Technology Enabled Care Services through the eyes of the individual.

Proven information from MIND on the five steps that can be taken for mental health wellbeing (Connect, Be Active, Take Notice, Learn and Give).

 

Manage your Health App

Mobile app developed by Keele University and Stoke on Trent Clinical Commissioning Group for managing your long term conditions. It also provides interactive activities and quizzes to test your knowledge.

 

Information about accessing care in your local area

Across the West Midlands there are many Local Authority hubs that provide information and signposting to local care services. Specific information and access to services can be found by accessing sites that support residents in Herefordshire, Worcestershire, Coventry, Warwickshire, Solihull, Birmingham, Wolverhampton, Shropshire and Staffordshire.

Commissioners are responsible for commissioning all services across health and social care.
