Urgent Care – Out of Hospital Programme
This Health Education England work programme focused on education and training to workforce in care homes, both residential and nursing, with the support of patient quality teams within seven clinical commissioning groups (CCGs) across Arden, Herefordshire and Worcestershire.
The programme aims to increase the awareness of and aid the early detection of the signs of a deteriorating patient. It ensures that staff have the skills, confidence and competence to appropriately deal with deterioration and avoid the need for hospital admission.
This programme was delivered from January to June 2016, with upport from Age UK Herefordshire and Worcetershire. The materials are open to all CCGs, Trusts and care homes (nursing or residential; registered or non-registered).
A programme evaluation has been commissioned and will be available in Autumn 2016.

Training Materials:
There are two modules of training for staff in care homes. All the materials were correct at time of publication and are available to all staff who attended the training and require a refresher. All new sessions of training should be reviewed by a clinical educator prior to dissemination.
- ’Essentials to Care’ (E2C) aimed at residential homes support workers, covering Nutrition and Hydration, Infection Prevention and Control, Catheter Care and Skin Integrity
This programme is part of a blended learning approach and the e-learning programme is available as Essentials in Care. - 'Recognising the Signs of the Deteriorating Patient' (RSDP) aimed at nursing homes; it helps registered nurses and senior health care assistants to understand the early indicators of deteriorating health, and observe, record and clearly relay to other health professionals
